School/District Tools
The tools listed below were collected during school site visits conducted in 2007 for the project described in our report Implementing Response to Intervention: Practices and Perspectives from Five Schools - Frequently Asked Questions, which can be downloaded from this site.
These PDF files are just a small sample of the types of forms, tools, and brochures that schools and districts were using in their RTI implementation at that time. All program, student, and school identifying information has been removed from these documents. We will add tools to this page periodically. More current tools may be found at the response to intervention (RTI) Web sites developed by the home states of our five sites: California, Florida, Oregon, Pennsylvania, and Wisconsin.
DISCLAIMER: The tools below have not been reviewed, vetted, or endorsed by the Center on Instruction or the U.S. Department of Education. They were developed as resources for use by the schools or districts we visited and were not intended to have a broader use. They are listed here simply as a way to fulfill a commonly received request for examples of tools that schools and districts use in their RTI implementation. Practitioners wishing to use these tools in their own schools or districts should consider modifying the tools to meet the unique needs of their educational setting(s).
